| Fertility evaluation involves both members of the couple. After a comprehensive history and physical exam, the basic evaluation involves looking at three areas of potential fertility problems: Ovulation, tubal or uterine disease, and male factor. Minimum testing includes tests for ovulation (ultrasound and hormone testing), a hysterosalpingogram (HSG: tubal dye xray), and a semen analysis (sperm count). After these tests are completed, either further testing will be warranted or treatment may be offered. For patients over 35 years of age or with certain characteristics, a Clomid challenge test is offered to test for age related ovarian dysfunction. In some cases, a laparoscopic surgery may need to be performed to directly visualize the tubes and ovaries looking for adhesions or endometriosis which would be appropriately treated at that time.
We perform on site semen analysis. Semen analysis is a specialized test and requires medical technologists who are knowledgeable in this area of laboratory technique. This test is especially sensitive to operator error and the difference between a normal and abnormal test is very slight. This is even more important for abnormal tests. It is critically important to define the degree of abnormality. For these reasons, we request that all patients have at least one semen analysis in our laboratory. See "Male Evaluation."
